MDS assessments failed to capture suicidal ideation

Summary

The facility failed to ensure Resident #2’s MDS assessments accurately reflected his status for suicidal ideations. The resident was a male with a history of stroke, hypertension, benign prostatic hyperplasia, diabetes mellitus, hyperlipidemia, thyroid disorder, and aphasia. His admission MDS showed a BIMS score of 14, and his quarterly MDS showed a BIMS score of 12. In both assessments, Section D was coded to show no interest or pleasure in doing things and no feeling down, depressed, or hopeless, while Section D.0150.i. thoughts that you would be better off dead, or hurting yourself in some way was not captured as positive. Record review showed that Resident #2 made suicidal statements during the period covered by the assessments. Nursing notes documented that he reported planning suicide by jumping out a window and/or wrapping cords around his neck, and he was transferred to the emergency room for suicidal ideation. Additional notes showed repeated suicidal statements, psychiatric referral, emergency evaluation, and transfer to psychiatric hospital services. The record also included a psychiatric assessment noting referral for depression, worrying, irritability, delusions, and suicidal/homicidal concerns, with the resident having been sent out for suicidal ideation with a plan. During interviews, SW B stated she coded the resident’s mood sections based on how he said he felt that day and did not review nursing notes to identify suicidal ideation within the prior 14 days. She stated she did not know she was supposed to ask about mood for the past 14 days for the MDS mood section. The DON and Administrator stated the resident had made suicidal statements and had been sent out for psychiatric evaluation multiple times, but the MDS assessments were still completed without capturing those suicidal statements in the mood section.
