Food Storage and Hot-Holding Temperature Deficiencies

Westover Hills Rehabilitation And HealthcareSan Antonio, Texas Survey Completed on 06-12-2026

Summary

The facility failed to store food in accordance with professional standards when surveyors found a box of taco shells in the dry storage area with a manufacturer's use-by date of 03/19/2026 and a box of frozen cookie dough in the freezer with the manufacturer's use-by date removed from the packaging. The cookie dough was labeled only with a handwritten date of 02/03/2026, and the Dietary Manager stated the handwritten date represented when staff opened the product. He confirmed the use-by date had been removed and said he could not determine the manufacturer's discard date, when the product should be discarded, when it was last used, or whether it remained suitable for use. The facility also failed to maintain hot-held foods at the required temperature during meal service. During observation, pureed bread measured 118 F and baked beans measured 121 F on the steam table, and the Dietary Manager stated the required hot-holding temperature was 135 F or higher. The Dietary Manager stated the food items would be reheated prior to service, and the items were removed from the steam table and reheated. The report also noted that dietary staff training records showed the Dietary Manager had current ServSafe Food Safety Certification and [NAME] A had completed ServSafe Food Safety training.
