Improper Dumpster Coverage and Refuse Disposal
Summary
The facility failed to dispose of garbage and refuse properly for two dumpsters that were observed during survey. On 6/22/2026 at 11:00 A.M., surveyors observed two dumpsters, with the dumpster on the right having both side doors open. The ground near the dumpster on the left had a dark residue near the base and debris on the ground, including a plastic straw, a plastic lid, a condiment packet, plastic waste, and paper waste. On 6/23/2026 at 12:12 PM, the plastic and paper trash seen the day before were gone, but the condiment packet remained on the ground. At that time, the dumpster on the left had one side door open, and the dumpster on the right had one side door open with a bag of trash overflowing from the opened side door. During interviews, CK A stated that the dumpster should be clean and covered and that dishwashers were responsible for keeping it clean and covered, while also stating that everyone uses the dumpster and that nursing staff used it but cooks did not. DW B stated that he checked the dumpsters every morning around 9:00 A.M. and again around 1:00 PM, and that the evening dishwasher checked them at 4:00 PM and 8:00 PM daily. The Dietary Manager stated that the struggle was keeping the tops and sides closed and sweeping around the dumpsters, and that there had been nursing trash around the dumpster that morning. The Administrator stated that dumpster lids should be closed when not in use for pest control purposes, and that not keeping dumpsters clean and covered increased the risk for pests.
