Infection Control and Hand Hygiene Failures During Medication Administration

Focused Care At Cedar BayouBaytown, Texas Survey Completed on 06-18-2026

Summary

The facility failed to maintain an infection prevention and control program for two residents reviewed for infection control. One resident had diagnoses including cerebral infarction, gastrostomy status, and aphasia, and had an active order for Enhanced Barrier Precautions due to the presence of a G-tube. Another resident had diagnoses including acquired hemolytic anemia, rheumatoid arthritis, anxiety disorder, discoid lupus erythematosus, and type 2 diabetes mellitus. During observation, an RN prepared to administer medications to the resident with a G-tube and entered the room where an Enhanced Barrier Precautions poster was displayed. The RN stopped the resident’s continuous feeding, lifted the resident’s gown, handled the G-tube extension, and attached a syringe for a medication flush before donning PPE. When asked about PPE requirements for medication administration through a G-tube, the RN stated it was not typically required if it was just a G-tube, then acknowledged the EBP poster and put on PPE from a cabinet in the room. In interview, the RN stated she had not received full orientation, had been placed on the floor on her first day, and had not been educated on EBP. During another observation, a second RN prepared medication for the other resident by removing a capsule from the medication bottle, placing it into her bare left hand, and then transferring it into a medication cup. After administering the medication, the RN returned to her cart and began charting without performing hand hygiene. In interview, the RN stated she had been trained in infection control and medication administration, acknowledged that medication should have been poured directly into a medication cup rather than placed in her hand, and stated she should have performed hand hygiene immediately after leaving the resident’s room. Other staff and leadership interviewed stated that EBP training had been provided, that PPE should be used for direct care with residents who have G-tubes or other indwelling devices, that medication should be poured into a cup, and that hand hygiene should be performed before and after resident care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.
