Summary

The facility failed to ensure that services provided or arranged for a resident met professional standards of quality when it did not obtain the ordered urology follow-up after a suprapubic catheter was inserted following hospitalization. The resident had a history that included stroke with right-sided weakness, dementia with agitation, neurogenic bladder, urinary retention, benign prostatic hyperplasia, recurrent urinary tract infections, and a suprapubic catheter placed after complications with an indwelling urinary catheter. The resident’s care plan and orders included suprapubic catheter care, monitoring, and dressing changes, but the record did not show that the ordered specialist follow-up was completed. Hospital discharge information reflected that the suprapubic catheter may need to be changed every 4 to 6 weeks, or as recommended by the provider. A nurse documented that a call was placed to the urologist to set up a follow-up appointment, but the office stated the resident’s Medicare Part A did not cover the appointment. Multiple staff interviews reflected that the follow-up was not completed because of payment or insurance issues. Staff also stated there was no clear order in place for when the suprapubic catheter should be changed, and the resident remained with the same catheter from insertion until the later hospitalization. The resident had a history of pulling on the catheter tubing and had previously been hospitalized for gross hematuria and a urinary tract infection after catheter-related problems. On 6/23/26, the resident was transferred to the hospital with lethargy, low blood pressure, and fever, and hospital records reflected diagnoses of septic shock and a UTI. The DON stated the resident had chronic urinary tract infections and that the facility did not know when the suprapubic catheter was supposed to be changed out. The Administrator later stated the appointment should have been expedited regardless, and the BOM stated the facility was responsible for the resident and should have arranged payment for the specialist visit.
