Late and Incomplete MDS Assessments for Two Residents
Summary
The facility failed to ensure that resident assessment data were completed and electronically transmitted within the required 14-day timeframe for 2 of 5 closed records reviewed for MDS transmission. For CR #10, a male resident with diagnoses including acute on chronic combined systolic/diastolic heart failure, end stage renal disease, and dependence on renal dialysis, the entry MDS dated 10/02/2025 required completion of the comprehensive MDS by 10/16/2025, but the comprehensive assessment was not completed until 11/12/2025, making it 27 days overdue. LVN A stated she was responsible for the comprehensive MDS and explained that the delay occurred because the resident had multiple readmissions and discharges during the stay, and she did not know whether he would return or remain out of the facility. For CR #25, a female resident with diagnoses including hemiplegia and hemiparesis following cerebral infarction, spastic hemiplegia affecting the right dominant side, and epilepsy, the record showed a completed discharge MDS and an incomplete admission MDS record dated 12/03/2025. LVN A stated she was responsible for the resident's entry and discharge MDS assessments and that she entered the admission record without realizing that leaving it incomplete started the 14-day completion requirement. During interview, the ADM and DON stated they were not aware of CR #10's untimely assessments or CR #25's incomplete admission record, and they expected MDS assessments to be completed within the required timeframe for regulation and billing purposes.
