Call lights not kept within reach for two residents
Summary
The facility failed to ensure call lights were within reach for two residents who were observed in bed without access to their call systems. Resident #6, a male with diagnoses including dementia, psychotic disturbance, mood disturbance, anxiety, type 2 diabetes mellitus with hyperglycemia, acquired absence of the left leg below the knee, severe protein-calorie malnutrition, pain in the right lower leg, and cognitive communication deficit, had a BIMS score of 07 indicating severe cognitive impairment. He used a wheelchair, required substantial assistance with personal hygiene and lower body dressing, and was always incontinent of bladder and bowel. Resident #6’s record also reflected safety precautions, including that the bed was to be in the lowest position and the call light within reach. During observation, he was found sleeping in bed with the call light out of reach, laying over a nightstand under a pillow, with the button draped down the side of the nightstand farthest from him. Staff interviews indicated the call light had not been properly reattached after care, and staff stated the call light needed to be within reach at all times. The DON and ADM stated residents’ call lights were to be within reach at all times and that CNAs and nurses were required to round every 2 hours and ensure call lights were in place. Resident #107, a male with diagnoses including diabetes mellitus, repeated falls, and mild cognitive impairment, had a BIMS score of 14 and used a walker for mobility. His care plan identified him as high risk for falls and included the intervention to be sure the call light was within reach and to respond promptly to requests for assistance. On two separate observations, he was lying in bed with his call light on the floor and out of reach, and he stated he could not reach it. Staff interviews confirmed call lights should be within residents’ reach and that staff should have noticed when the call light was clipped to the bed or otherwise out of reach.
