Summary

The facility failed to provide and document sufficient preparation and orientation to ensure a safe and orderly transfer or discharge for one resident. The resident had multiple diagnoses including cerebral infarction, right hip fracture status post hemiarthroplasty, compression fracture of T1, end stage renal disease with dialysis three times weekly, type 2 diabetes, congestive heart failure, atrial fibrillation, hypertension, and repeated falls. His discharge MDS showed a BIMS score of 15, and the record also identified Spanish as his preferred language and noted that he stated he needed an interpreter to communicate with health care staff. Care conference documentation showed discharge planning discussions with the resident and his representative, including discussion of discharge location, PCP, pharmacy, DME, home health, and transportation. However, the record also showed that the social worker was unable to reach the family representative for some meetings, and the social services note on the day of discharge documented an attempt to contact the representative about discharging the resident into a group home, with a text message sent because voicemail was full. The physician order stated the resident could discharge home with HHC of choice and be evaluated for PT/OT/ST and skilled nursing as needed, while the discharge plan of care listed the resident as an assist of one. Interviews and records showed conflicting accounts of the discharge arrangement and the placement itself. The resident and family representative stated they were not informed in advance that the resident would be discharged that day and believed the group home would be similar to a smaller nursing home with nursing staff. The resident was transported by EMS via stretcher to a boarding home/group home setting, and the director of that home stated there were no nurses on staff, the caregiver only had CPR certification, and the home was actually a boarding house where residents were independent. The director stated the resident arrived unable to walk or communicate in English, and the family representative removed him the same day after the resident called crying and frightened. The DON stated she did not investigate the status of the placement to ensure it was safe, and the discharge form signed by the resident was not documented in nursing notes or elsewhere.
