Summary

The facility failed to establish and maintain an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of communicable diseases and infections for 1 of 6 residents reviewed for infection control practices. Resident #1 was an older male admitted with a primary diagnosis of unspecified dementia without behaviors. His admission MDS reflected clear speech, severe cognitive impairment with a BIMS score of 3, and that he usually required substantial to maximum assistance with toileting hygiene and was always incontinent of urine. His care plan stated that he required maximum assistance with ADL care and was incontinent of bowel and bladder, with interventions to provide incontinent care every two hours as needed. During an observation of incontinent care, CNA A and CNA B provided care to Resident #1. CNA A removed the brief, wiped the front peri area, turned the resident to his side, wiped the back peri area, applied barrier cream, and placed a clean brief without changing gloves or performing hand hygiene. During interview, CNA A stated she normally changed gloves and performed hand hygiene during incontinent care and acknowledged she should have changed gloves and performed hand hygiene when moving from the front to the back and after applying barrier cream. CNA B stated she observed CNA A not changing gloves or performing hand hygiene at the appropriate times. The DON, who was also the infection control preventionist, stated she expected staff to change gloves and perform hand hygiene during incontinent care when moving from front to back or from clean to dirty. The Administrator also stated she expected staff to practice safe infection control during incontinent care. The facility's Perineal Care policy did not address changing gloves or performing hand hygiene.
