Food Service Failed to Provide Palatable, Appetizing Meals
Summary
The facility failed to ensure that food and drink were palatable, attractive, and served at a safe and appetizing temperature for residents who ate orally from the only kitchen in the facility. Surveyors observed and residents reported that meals were frequently cold, poorly presented, and missing items listed on the menu or meal ticket. The deficiency was supported by resident interviews, meal observations, grievance records, and resident council minutes documenting repeated complaints about temperature, taste, portion size, and missing condiments. Resident #52, who had type 2 diabetes and major depressive disorder with a BIMS score of 10, stated that meals were always the same, the food was cold, and oatmeal was too runny. Resident #49, who had sepsis and type 2 diabetes with a BIMS score of 12, stated that breakfast oatmeal was frequently too watery. In a confidential group interview, 10 of 14 residents reported that breakfast was cold, condiments were not on the tray, desserts did not match the ticket, food was too salty or had no seasoning, and they often did not know what they were going to be eating. Residents also stated they did not receive enough food and sometimes got seconds while other times they did not. Resident #37, who had type 2 diabetes with diabetic CKD, anemia, gout, and a BIMS score of 7, stated that the chicken fajita meal was salty, lacked dressing or salsa, was not sufficient, and did not include the apple enchilada listed on the menu. Resident #95, who had type 2 diabetes with neuropathy, anemia, hypertension, and a BIMS score of 12, stated the charro beans were hard and not the right kind of beans, the portion was not enough, and the meal did not include guacamole listed on the menu. The observed test tray for the lunch meal contained a soft taco with chicken fajita, a salad, charro beans made with chickpeas, iced tea, water, and a cookie, but no guacamole, picante sauce, salad dressing, salt, pepper, sugar, or apple enchilada. The tortilla was cold, the chicken fajita was lukewarm, and the tray was described as unappetizing in appearance. The kitchen staff stated condiments were not available outside the kitchen and residents relied on dietary staff to place them on trays. The cook stated he had not tasted the food he prepared and was not aware of any negative outcome from unpalatable food. The social worker stated food grievances were the biggest resident complaint and included portion size, taste, texture, and food being cold, but she had not escalated the concerns to the Administrator. The Administrator, who was acting as Dietary Manager, stated he expected food to look and taste good, acknowledged the chickpea substitution for pinto beans was not a good substitution, and stated there was no policy on food palatability. Grievance records and resident council minutes documented repeated complaints about missing condiments and meals not being at a satisfactory temperature.
