Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards in 1 of 1 kitchen. During an observation in the walk-in refrigerator, surveyors found undated and unlabeled hamburger patties in an aluminum container, 2 pounds of undated sliced smoke turkey breast in a resealable food storage bag, 3 undated chicken salad sandwiches in a resealable food storage bag, 2 resealable plastic containers of chicken salad dated 06/14/26, 1 resealable plastic container of chili dated 06/20/26, sliced pickles dated 06/23/26, and 1 resealable plastic container with beans dated 06/24/26. During interview, dietary staff stated all staff were responsible for checking expiration dates, that the date on bags and containers was the preparation date, and that leftovers were good for three days before needing to be thrown out. The Dietary Manager stated all kitchen staff were responsible for labeling and dating refrigerated items and that leftovers had to be disposed of by the third day. During a later observation, puree meat, puree vegetables, and sauce were placed on the steam table with no tops, and regular meat and regular vegetables were placed on the steam table with tops. The Dietary Manager began plating residents' lunch before checking the temperatures of the food on the steam table. In interview, the Dietary Manager stated she checked temperatures before the food was placed on the steam table, then stated the proper way was to check the temperature on the steam table before service to residents. Record review showed the facility policy required leftovers to be used within 72 hours and discarded if over 72 hours old, and the Taking Temperatures policy required temperatures of all foods to be taken and recorded prior to service.
