Food Prep and Tray Distribution Hand Hygiene Failures

Summary

The facility failed to store, prepare, and distribute food in accordance with professional standards for food service safety during kitchen sanitation observations. During an observation, DA H was standing over the food prep table portioning salads onto individual plates without wearing a hair net, and she later stated she had forgotten to use a hair net while preparing food in the kitchen. She also stated she had forgotten to wash her hands after removing her gloves. During meal distribution observations, LVN A reviewed a meal ticket at the tray distribution cart, walked across the dining room to observe a resident's meal ticket, then returned to the cart and resumed distributing trays without using hand sanitizer between tray service. CNA C entered the dining room, shook bare hands with LVN B, and then served a tray of food to a resident without washing his hands or using hand sanitizer. LVN B had hand-to-hand contact with CNA C while greeting him, then failed to use proper hand sanitation after that contact and before verifying meal tickets for accuracy. Both CNA C and LVN B stated they had been trained in hand hygiene. During another observation, designated hand hygiene station #2 did not have hand soap, paper towels, or a designated trash can available for staff use between kitchen tasks. In addition, [NAME] prepared puree spinach, removed gloves, brought a pan of rice to the prep area, and failed to wash hands before putting on a new pair of gloves. The DM, RD, and ADM were interviewed regarding the observed practices, and record review showed staff training in hand hygiene, food handler safety, and facility policies requiring hair restraints, proper hand-washing, and sanitary food preparation practices.
