Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards in the kitchen and satellite kitchen. During observation of the main kitchen, a Dietary Aide was seen wearing gloves while rinsing dirty dishes and loading dish racks for the dish machine, and he stated he washed his gloves before touching clean dishes. The Culinary Bench Manager told him he could not wash his gloves and needed to remove them, discard them, and put on a new pair. The culinary agency Regional Director of Operations later stated staff had been in-serviced regarding handwashing and glove changes in the dish room, and that if the dish washer washed his hands between handling soiled dishes and putting away clean dishes, he would not need to wear gloves. The walk-in freezer contained open cardboard boxes/cases with bagged food items exposed to the air, including one box of enchiladas and two boxes of meat patties. The deep fryer units had dried fried food crumbs and breading crumbs on the interior surfaces. A reach-in refrigerator contained a square plastic container labeled and dated as lemon pudding 3/02/2026, but the lid was warped and did not securely fit the container. A stainless steel shelf with a mesh liner had food crumbs and dust beneath the shelf liner. In the non-perishable food storage room, a row of 6.56 pound cans of tomato soup had not been dated when delivered, and an open 10 pound bag of dry spaghetti noodles had been rolled closed and placed in a plastic bag that was too small to seal; there was also an open package of tri-colored spiral pasta wrapped with plastic wrap. The second-floor satellite kitchen had a handwashing sink with hot water and paper towels, but no handwashing soap and no waste basket near the sink. The ice machine top exterior surface was lightly soiled with dust, and an empty coffee cup was resting on top of the ice machine. The facility policy for dry goods required packaged and canned food items to be kept clean, dry, properly sealed, neat, and date marked as appropriate, and the cold food policy required food items to be stored in covered containers, labeled and dated, and arranged to prevent cross contamination.
