Summary

The facility failed to ensure menus were followed and failed to post the current week's menu in the dietary department and in a location visible to residents. During an observation, the menu posted outside the dining room was identified as the week 4 menu and listed lunch as cheese quesadillas, garden blend vegetables, refried beans, sugar cookies, and dinner as hamburger on a bun with tossed salad, lettuce, tomatoes, pickle, potato wedges, and peach cobbler. However, the posted dinner entry also reflected a different version of the meal, and the menu in the kitchen was a week 3 menu. Staff stated the posted menu was hardly ever right, and the dietary cook stated he cooked what was posted in the kitchen by his manager and did not know what week it should be. At lunch, the plates observed contained lasagna, roll, cake, and salad instead of the posted cheese quesadillas, garden blend vegetables, refried beans, and sugar cookies. The resident interviewed stated staff sometimes gave the wrong food item or were out of what he wanted, and that staff came into his room daily to write down what he wanted for meals. The resident had diagnoses including chronic ischemic heart disease, adult failure to thrive, unspecified severe protein-calorie malnutrition, heart failure, generalized anxiety disorder, bipolar disorder, and a neoplasm of uncertain behavior of the meninges. The ADON stated kitchen staff were responsible for updating and posting the menu, and the DON stated the dietary department was contracted and did not think there were negative resident effects because staff informed residents of meals daily.
