Incomplete Blood Pressure Documentation for Midodrine Administration

Summary

The facility failed to maintain complete and accurately documented medical records for Resident #47 by not documenting blood pressures on 13 separate occasions in May 2026. Resident #47 was a [AGE]-year-old male admitted with diagnoses of cerebrovascular disease, essential hypertension, and hypotension, and he had an order for Midodrine 10 mg by mouth twice daily for hypotension, with instructions to hold the medication if blood pressure was greater than 110/60. The physician order summary also included vital signs every month and every day and night shift every month. Record review showed no blood pressure documentation on the dates and times Midodrine was administered, and the vital sign log also had no blood pressure entries for those same occasions. The care plan identified that the resident had hypotension and that medication should be given as ordered while monitoring for side effects and effectiveness. During interview, the LVN stated blood pressure was taken before administering Midodrine but also stated she sometimes gave the medication when blood pressure was outside the ordered parameters depending on the resident and how the medication would affect blood pressure. The DON stated nurses and med aides should take blood pressures before administering blood pressure-altering medication and that the facility had not monitored documentation in the past 2 weeks because of short staffing and other issues.
