Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards for food service safety in the kitchen and nutrition room. During observation, the entire kitchen floor was slick, the ice cream freezer had approximately 1 1/2 inches of ice build-up on all four walls with removable brown/black substance on the seals, and the ice machine door was nearly falling off with a removable black substance on the ice chute. The gas ovens were not in use, but the handles were heavily coated with a sticky substance, and the area behind the stove contained roach traps covered with roaches, a rotting tomato, and a film of grease and dirt on the baseboards, floor, and electrical conduit. The observation also found an unlabeled 16-ounce bottle of water in the back of refrigerator B, a handwashing sink with no hot water and no cold water when turned on, and a paper towel dispenser that did not work. Five of five steam table wells had a flaking yellowish substance covering the bottoms and up the sides. In the walk-in freezer, boxes were stored less than 18 inches from the ceiling, and a 1-gallon zip-type bag of cheese was unsealed and open to air. In the nutrition room, there was expired enteral feeding present. Interviews and record review showed staff awareness of some of the problems but also showed that cleaning and maintenance were not being carried out as documented. The DM stated staff were checking off cleaning schedules without doing the work and that she was not checking on them as often as she should. The MS said he was unaware of the oven not working and that the ice machine door had been damaged for a while. The RD stated she was aware of sanitation issues from a recent audit and noted that boxes in the freezer were too close to the ceiling. The daily and weekly cleaning schedules for the kitchen were signed as completed, even though the observed conditions showed the ovens, hand sink, ice machine, freezer, steam table, and other areas were not maintained in clean and sanitary condition.
