Pest Control Program Failed to Keep Resident Rooms Free of Rodents
Summary
The facility failed to maintain an effective pest control program to keep resident rooms free of rodents. During the survey, two residents were identified as having repeated sightings of mice in their rooms, and both rooms contained numerous black oblong objects that were observed and identified by staff and the pesticide operator as mouse droppings. One resident had lived in the room for about five months and stated that she had seen mice several times, including during a therapy treatment when a mouse ran across the room and into her closet. The other resident stated that mice had come in from under her room door and into her closet over the past few months. Record review showed that one resident was cognitively intact and required assistance with toileting, showers, dressing, and personal hygiene, while the other resident was also cognitively intact and required partial/moderate assistance with bed mobility and toilet transfer. The facility grievance log documented a complaint from another resident who reported seeing a rodent in her room, and the pest control sighting log documented multiple rodent sightings over several weeks. A service inspection report noted trap inspections in the kitchen and in the building where the affected residents lived for mouse activity. During interviews, the maintenance manager stated he had received complaints about mice, had set out glue traps in some rooms, and had caught a few mice, and he also reported seeing a live rodent in a resident room about 10 days earlier. The pesticide operator stated he had been aware of rodent sightings in the facility for about 6 months, had caught a few mice a couple of weeks earlier, and had placed traps inside the facility. The DON stated she had not been alerted by staff about mice in the building, and the facility policy stated that it maintained an ongoing pest control program for insects and rodents.
