Infection Control Failures During Wound, Catheter, and Incontinent Care

Summary

The facility failed to maintain an infection prevention and control program for three residents reviewed for infection control. Resident #1 had quadriplegia, a neurogenic bladder with an indwelling catheter, diabetes, and unhealed pressure ulcers. His care plan indicated enhanced barrier precautions (EBP) were required for high-contact care activities and device care, including catheter care and wound care. During observation, CNA B and CNA D provided incontinent care and catheter-related care without wearing gowns, and CNA D handled the catheter bag and tubing without gloves or a gown. CNA B later provided additional incontinent care, used the same wipe multiple times during perineal care, and did not perform hand hygiene after removing gloves. The ADON then performed wound care while repeatedly removing and replacing gloves without hand hygiene between glove changes, and the treatment nurse also performed wound care using the same gauze repeatedly during cleansing. Resident #2 had dementia, was dependent on staff for all ADLs, and had wounds with an order for wound treatment to the right first metatarsal. His care plan indicated EBP were required for wound care, and the ADON stated he also required contact precautions because he had ESBL and MRSA in his wound. Video observations showed RN F entered the room and provided wound care twice without wearing a gown. On observation, the room had an EBP sign on the doorframe, but there was no sign indicating contact precautions. RN F stated she did not wear a gown because she did not realize the resident required contact precautions and said there was no sign on the door to alert staff and visitors. Resident #4 had spinal stenosis and a deep incisional surgical site infection, with a wound vac ordered for the lower back wound. Her care plan did not address EBP, and there were no orders for EBP. During observation, LVN N entered the room to reconnect the wound vac without wearing a gown or gloves. She handled the wound vac and tubing with ungloved hands, wiped the tubing tip with an alcohol swab, and touched the resident’s linens and wound vac tubing while attempting to reconnect the device. LVN N later stated that EBP should be followed when a resident has wounds and that she forgot to put on a gown and gloves.
