Summary

The facility failed to document the post-dialysis assessment for a resident who received hemodialysis, leaving the Dialysis Communication Forms incomplete on multiple occasions. Resident #7 was admitted with end stage renal disease, hypertensive heart disease with chronic kidney disease, heart failure, and diabetes mellitus. His orders included hemodialysis every Monday, Wednesday, and Friday, monitoring of the dialysis access sites, and weighing before and after treatment with communication to office staff. Record review showed that the Dialysis Communication Forms for May 2026 were incomplete on 5/04/26, 5/08/26, 5/13/26, 5/18/26, and 5/20/26. On those dates, the post-dialysis section did not contain documentation of vital signs, any new orders, or assessment of the dialysis access site for swelling, redness, signs of infection, or whether dressing reinforcement was needed. The forms showed the pre-dialysis portion had been completed by RN A on several of those dates, but the post-dialysis portion was left blank. During interviews, RN A stated that after the resident returned from dialysis, the nurse on duty was responsible for checking vital signs, assessing the access site, and completing the bottom portion of the form. The ADON and DON both stated that the nurse on duty was responsible for completing the post-dialysis section when the resident returned, including monitoring for stability and any adverse effects after dialysis. The facility policy also stated that community staff would complete the post-dialysis section and assess the resident, then place the document in the medical record.
