Pharmacy Orders Entered Without Provider Authorization
Summary
The facility failed to provide pharmaceutical services that ensured the accurate acquiring, receiving, dispensing, and administering of medications for five residents reviewed for pharmacy services. The deficiency involved Mounjaro being entered and processed for residents without corresponding provider orders, with some shipments already delivered to the facility. The report states that this created a concern involving medication ordering accuracy for residents with diabetes diagnoses and varying levels of cognitive impairment. Resident #4 was a female with type 2 diabetes mellitus, moderately impaired cognition, and no insulin use. Her record showed Mounjaro orders entered by nursing staff on two occasions, including one order that was later revised and shipped by the pharmacy and another that resulted in a one-month supply being shipped. The email from the FNP stated that one order had been cancelled in PCC after it had already been processed, and the resident was not able to answer questions regarding injection medications. Resident #5 was a female with type 2 diabetes mellitus and impaired cognition. Her record showed a Mounjaro order entered under an LVN’s name, with the medication shipped to the facility before the order was cancelled in PCC and later discontinued by the FNP. A witness statement from the LVN stated that someone went under her PCC profile for the resident and that she was not at the facility when the order was placed. Resident #6 was a male with type 2 diabetes mellitus, moderately impaired cognition, and use of insulin and other diabetes medications; his record showed a Mounjaro order entered by an LVN and shipped by the pharmacy before being cancelled. Resident #7 had type 2 diabetes mellitus and impaired cognition, and the record showed multiple Mounjaro orders entered and later cancelled, with the pharmacy not shipping the medication because insurance denied it. Resident #8 had type 1 diabetes mellitus and intact cognition; her record showed a Mounjaro order entered by an LVN, with the pharmacy denying shipment due to insurance and the order later cancelled in PCC. The Administrator stated that staff had saved passwords on computers, allowing someone to log in using another staff member’s credentials and order Mounjaro for residents who were not taking the medication.
