Summary

The facility failed to ensure a resident with pressure injuries received necessary treatment and services to promote healing, prevent infection, and prevent new ulcers from developing. The resident had diagnoses including MS, polyneuropathy, heart disease, and a history of MI, and the MDS indicated severe cognitive impairment, fluctuating inattention and disorganized thinking, functional limitation in both lower extremities, dependence for most ADLs, and bowel and bladder incontinence. The resident was also on hospice services and was identified as being at risk for pressure ulcers/injuries. The resident’s care plan included turning and repositioning frequently, keeping skin clean and moisturized, weekly full-body skin inspections, a pressure-reducing mattress, and ordered skin/wound treatments. Physician orders included boot-style heel protectors on both feet at all times and wound care to the buttocks. Skin assessments documented progressive breakdown, including redness to the sacral area, redness to the right heel, a scab to the left heel, and later open areas and wounds to the sacral area and buttocks. Progress notes also documented a stage 2 pressure injury to the buttocks. During multiple observations on the same day, the resident was lying on her right side with her legs pulled up, knees and feet resting on each other, and her heels pulled up to her buttocks, with the left heel touching the lower right buttock. There were no pillows or positioning devices between the knees, ankles, or feet, and the heel protector boots were found on top of a dresser rather than on the resident. The resident had multiple reddened areas and dark areas to both feet and heels, including areas where skin appeared missing. Staff interviews indicated the resident was not being consistently repositioned, the boots were not consistently in place, and no cushioning was being used between bony prominences. The treatment nurse stated she was only providing treatment to the wound on the resident’s bottom and was not providing care to the feet.
