Summary

The facility failed to establish and maintain an infection prevention and control program for two residents reviewed for infection control. One event involved a CNA providing incontinent care to a resident with bowel and bladder incontinence and moderate cognitive impairment. During observation, the CNA used a wipe that had already contacted the resident’s bottom to clean the perineal area again, and this occurred three times. The CNA later stated she was not aware she had used a soiled wipe again and acknowledged the action could cause an infection. A second event involved a medication aide administering medications and checking blood pressure for two residents with hypertension. One resident had severely impaired cognition and orders for antihypertensive medications, and the other resident had a BIMS score of 13 with orders for lisinopril, metoprolol, and diltiazem. During observation, the medication aide used the same blood pressure cuff for both residents, placing it on the medication cart between uses, and did not sanitize the cuff after checking the first resident’s blood pressure or before using it on the second resident. Interviews confirmed the facility expected the blood pressure cuff to be sanitized after use. The CNA, RN, DON, and ADON all acknowledged that soiled wipes should not be reused and that the BP cuff should be sanitized immediately after checking a resident’s blood pressure to prevent spread of infection. The facility policy stated medication administration requires universal precautions and proper hand hygiene, and the infection control policy stated the facility will establish and maintain an infection control program designed to provide a safe, sanitary, and comfortable environment and help prevent the development and transmission of disease and infection.
