Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards for food service safety in 1 of 1 kitchen reviewed. During the initial kitchen tour, surveyors observed a bag of red grapes in the refrigerator that was not fully sealed, hamburger buns in the pantry that were dated and left on the shelf, a large container of brown sugar in the pantry that was not fully sealed, and a box/bag of cookies in the freezer that was not fully sealed. Surveyors also observed a dried white substance on the side of the food preparation table and black unknown substance on the inside area of the dining room ice machine where ice was dispensed. On return observation, surveyors noted a black substance on the bottom half of the wall by the back door and sink and a fire extinguisher by the microwave with a thick layer of dust. The DM stated the grapes were not fully sealed because she did not check them when placing them in the bag, the cookie box/bag was opened from the wrong side, the hamburger buns had not yet been addressed in the pantry, and the brown sugar had been used for breakfast. The DM also stated she did not know what was on the food preparation table, that the ice machine had last been cleaned when the Health Department was in the facility, and that she was responsible for keeping up with cleaning of the ice machine, kitchen walls, and ensuring food was properly sealed or removed from shelves when no longer good to use.
