Failure to Submit PASRR Specialized Services Request
Summary
The facility failed to coordinate assessments with the PASARR program to the maximum extent practicable for 1 of 3 residents reviewed for PASARR services. Resident #1 was a [AGE]-year-old male with diagnoses including quadriplegia, neuromuscular dysfunction of the bladder, major depressive disorder, anxiety disorder, and spondylolisthesis. His quarterly MDS showed a BIMS score of 15 out of 15, indicating intact cognition, and he used a wheelchair for mobility. His care plan identified him as PASRR positive due to a history of developmental disabilities and noted that he received habilitation coordination, CMWC, and independent living skills. Record review showed that Resident #1 had a PASRR Comprehensive Service Plan dated 3/16/2026 in which LA-IDD documented agreement to customized wheelchair, habilitation coordination, and independent living skills. After the resident’s change in condition when he was taken off hospice, the facility held a new initial IDT meeting on 3/16/2026. However, the facility did not submit a complete and accurate request for nursing facility specialized services in the LTC Online Portal within 20 business days after that IDT meeting. The PASSR Program Specialist stated the request had not been submitted within the required timeframe and that an email had been sent requesting the information be uploaded, but it still had not been uploaded. The Social Worker stated she had not uploaded the IDT service requests to the portal and believed the LIDDA coordinator usually handled that task. She also stated she was not familiar with where the requests should be uploaded and had not done this before with other PASSR residents. Email correspondence showed the Social Worker forwarded the state agency notice of non-compliance to the LIDDA coordinator asking for assistance, and the coordinator responded that the resident was currently receiving habilitation coordination, independent living skills, and a customized wheelchair. The Administrator stated there was confusion because the resident had already been receiving services and had obtained the motorized wheelchair through insurance after leaving hospice, but the facility’s PASSR rules guidelines stated that specialized services are initiated by submitting the request to the agency within 20 days of the PCSP meeting.
