Failure to Submit PASRR Specialized Service Request for Customized Wheelchair
Summary
The facility failed to ensure that a resident identified as PASRR positive received specialized equipment in accordance with the PASRR Comprehensive Service Plan and failed to submit a request for specialized services in the LTC Online Portal. The resident was an older male with diagnoses including moderate intellectual disabilities, dementia, anxiety, major depressive disorder, delusional disorders, dysphagia, hypertension, and osteoarthritis. His quarterly MDS showed severe cognitive impairment with a BIMS score of 02, communication deficits, impaired vision and hearing, and significant functional dependence, including the need for a wheelchair for mobility and extensive assistance with transfers, toileting, bathing, dressing, and wheelchair mobility. Record review showed that the resident’s care plan identified him as PASRR positive and stated that specialized services determined necessary by the IDT would be initiated and a request submitted within 20 business days after the IDT meeting. The PASRR Comprehensive Service Plan documented that the IDT reviewed the plan of care and determined that he would benefit from a customized wheelchair. During interview, the PASSR Representative stated that the meeting had been held and that the facility still had not submitted the request for authorization of the customized wheelchair. Facility staff gave conflicting accounts about the wheelchair request. The Social Worker stated she believed the wheelchair had been ordered but had not followed up with therapy. The DOR stated he thought it had been ordered, but he was unsure how it was processed and acknowledged he did not follow up with the medical equipment supplier; he also stated he later learned the resident did not have applied income, which was why the wheelchair was not ordered through Medicaid and that the request should have been submitted through the PASSR program. The BOM confirmed the resident did not have applied income, and the DON and Administrator stated the resident was currently using a standard wheelchair and that staff had a knowledge deficit regarding the PASSR program. The facility policy stated that if a resident is positive, specialized services are to be initiated by submitting a request within 20 days of the PCSP meeting.
