Hoyer Lift Not Sanitized After Resident Transfer

Foothill Heights Care CenterPasadena, California Survey Completed on 06-04-2026

Summary

The facility failed to ensure standard infection prevention and control practices were followed when a hoyer lift used for Resident 1 was not sanitized after the transfer. Resident 1 was admitted on 1/18/2024 and readmitted on [DATE], with diagnoses including a disorder involving the immune mechanism. The resident’s MDS dated 4/20/2026 indicated severely impaired cognitive skills for daily decision making and dependence for multiple activities of daily living, including toileting hygiene, bathing, lower body dressing, and bed-to-chair transfers. During an observation on 6/4/2026 at 12:20 PM, two CNAs transferred Resident 1 from a geri chair to bed using a hoyer lift while the resident had Enhanced Barrier Precautions signage posted at the door. During a concurrent observation and interview on 6/4/2026 at 12:35 PM, CNA 1 was observed taking the used hoyer lift into the hallway and plugging it into a power outlet without sanitizing it. CNA 1 stated he did not wipe the hoyer lift with sani wipes because he did not find them on the PPE cart outside the room. Later that day, CNA 1 confirmed he did not sanitize the hoyer lift after using it and stated he knew it needed to be sanitized before and after each resident use. The Infection Prevention Nurse reviewed the facility policy titled Lifting Machine, Use of Mechanical and stated it indicated lift surfaces were to be disinfected with sani wipes. The Administrator also reviewed the policy and stated it indicated to disinfect lift surfaces, but did not specify before and after use.

E
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident with an indwelling Foley catheter had EBP identified in the care plan, but staff did not consistently wear gowns during close contact care, including vital signs, medication administration, hygiene-related contact, and topical treatment. In addition, a TMA administered medications to three residents without sanitizing hands between residents or before handling medications, despite policy and DON expectations requiring hand hygiene. Laundry practices were also inconsistent with sanitary handling, as staff sorted soiled laundry without gowns and gloves being available in the room and reported using only gloves for most dirty laundry tasks.

The DON stated both actions were a break in infection control, and the facility policy required proper hand hygiene and noted that gloves do not replace hand hygiene.
