Summary

The facility failed to provide a safe and sanitary environment for three sampled residents by not ensuring that humidifier bottles and oxygen tubing were dated when changed. During an observation in the room shared by two residents, the Registered Nurse Supervisor stated the humidifier bottles and tubing were not dated and explained that if they are not dated, staff would not know when to change them. The supervisor also stated the humidifier bottles and tubing should be changed weekly, and that if they are not changed, organisms could grow and the resident could be at risk for infection. One resident had diagnoses including sleep apnea, diaphragmatic hernia, and epilepsy, and was documented as having the capacity to understand and make decisions. The other resident had chronic respiratory failure, a history of pneumonia, and a tracheostomy, and was documented as lacking decision-making capacity and being severely cognitively impaired and dependent on staff for multiple ADLs. The facility also failed to keep the room of another resident free of flies. That resident had chronic respiratory failure, a tracheostomy tube, a gastrostomy tube, type 2 DM, and was documented as lacking decision-making capacity, being severely cognitively impaired, non-verbal, and bedbound. During observation, a housefly was seen in the room and landed on the resident's right hand and left cheek near the mouth while the resident had the mouth wide open. During a later interview, the Respiratory Therapist stated houseflies are known to carry bacteria, viruses, and potential parasites and could cause infections to residents with tracheostomy and gastrostomy tubes by laying eggs and causing maggots to form in the openings. The facility's infection control policy required a safe, sanitary, and comfortable environment, and the pest control policy required an ongoing pest control program to keep the building free of insects and rodents.
