Improper Food Labeling and Cold Holding Temperatures

Imperial Crest Health Care CenterHawthorne, California Survey Completed on 05-22-2026

Summary

Food safety and sanitary food storage and preparation practices were not maintained in the kitchen. During observation and interview, one container of Italian dressing was found stored in the kitchen refrigerator without an opened date or use-by date. The Registered Dietitian stated that food items should be labeled with the date opened and use-by date so staff can determine when the item was opened and when it is no longer safe for consumption. The facility's Food Receiving and Storage policy stated refrigerator foods must be labeled, dated, and monitored to ensure use by their use-by date. During lunch service, three-bean salad and vanilla mousse were observed at temperatures above the facility's required cold-holding standards. One pre-plated cup of three-bean salad measured 50 F, and another cup held in an ice-holding bin measured 66 F. Bowls of vanilla mousse were also observed in an ice-holding bin with ice that did not fully surround the bowls, and three randomly selected bowls measured 50 F. The RD stated the three-bean salad and vanilla mousse were not maintained at 41 F or below, and the facility's Menu Service policy stated salads and desserts must be served at 40 F or below.

The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
