Summary

The facility failed to follow professional standards of practice for one resident when the resident’s PET/CT scan preparation was not carried out as ordered. The resident was admitted with diagnoses including STEMI and diplopia, and the MDS dated 4/28/26 indicated intact cognition. The appointment instructions for the PET/CT skull to mid thigh required the resident to have nothing to eat for 6 hours before the appointment, and the physician order also directed the resident to check in for the PET scan at 10:00 a.m. on 5/29/26. The resident’s nutrition record showed the resident ate 26-50% of a meal at 9:00 a.m. on 5/29/26 and also ate 26-50% of an alternative meal at that time. The nurse progress note stated the resident returned from the appointment and the appointment was not completed because the resident had multiple bowel movements at the appointment, and the note also stated the resident was to be NPO 6 hours ahead. During interview, the DON confirmed the resident had been expected to not receive food before the procedure and confirmed that if the resident received meals at 9:00 a.m., then the order was not followed. The facility also failed to follow the physician-ordered hospital destination for the resident’s transfer. The physician order directed staff to send the resident to Hospital A, and the transfer form documented the reason for transfer as new onset vision change and indicated the resident was sent to Hospital B instead. The ISW stated the outpatient physician ordered the resident to be sent to Hospital A ED, but the facility sent the resident to Hospital B instead, and staff were unable to tell where the resident was transported. The LN and DON both confirmed the order was for Hospital A, the transfer form showed Hospital B, and there were no progress notes documenting the transfer location.
