Summary

The charge nurse stated elopement risks were discussed in huddles and that he asked the assigned CNA to watch the resident before leaving for lunch, but he did not review elopement care plan interventions before the incident. CNAs stated they were unaware the resident required increased supervision or had wandering concerns. The DON stated the resident should have had a care plan initiated after the June 5 assessment and that it was not implemented until after the June 14 incident. The facility policy stated residents identified as at risk for wandering or elopement should have care plan strategies and interventions to maintain safety.
