Summary

The facility failed to ensure that Resident 29 received enteral feeding as ordered by the physician. Resident 29 was admitted with diagnoses including metabolic encephalopathy, injury of the left foot, and major depressive disorder. The physician’s order dated September 21, 2025 directed enteral feeding with [Brand name] Formula 1.2 at 51 mL/hr for 18 hours, or until the dose was completed via GT, to provide a total of 918 mL per day and 1102 kcal, starting at 2:00 PM. During observation on June 15, 2026, Resident 29 was lying in bed and receiving enteral feedings, and the formula bottle was observed infusing with a date of June 14, 2026 and a start time of 1400. At 3:45 PM, LVN 5 verified the amount remaining in the bottle and stated Resident 29 had received approximately 670 mL over one day, one hour, and 45 minutes, which was less than the ordered 918 mL per day. During interview and record review, the DON reviewed the facility’s Enteral Nutrition policy and stated the policy was not followed, and that it was important to follow the physician’s enteral feeding order to help meet Resident 29’s nutritional needs.
