Summary

The facility failed to ensure adequate nutritional interventions and monitoring for two residents with weight loss. The weight record showed a downward trend from 128.4 lbs. to 108.4 lbs. over several months, but the Weights and Vitals Summary did not contain documented weekly weights from the time the order was in place through the date of review. The DON stated the weights should have been documented in the electronic medical record and acknowledged the policy was not followed.
