Summary

Resident 8 was admitted with diagnoses including kidney transplant, neuromuscular dysfunction of the bladder, generalized muscle weakness, and gastrostomy status. The resident’s order summary indicated an indwelling foley catheter for neurogenic bladder with daily checks for placement and patency, and the care plan directed foley catheter care with soap and water every shift. During interviews, CNA 1, LVN 3, the treatment nurse, the infection preventionist, and the DON described that nursing staff were expected to clean the catheter with soap and water and document the care. Review of the nursing notes and TAR for the relevant period did not show that staff cleaned the catheter. The facility’s policy for catheter care required routine cleansing of the catheter from the insertion site outward and documentation of the date, time, person providing care, assessment data, and the resident’s tolerance, but the DON stated there was no documentation that the foley catheter care had been completed and that the policy was not followed.
