During the observation, the faucet was seen unattached, and the sink did not have hot water available for use. The Housekeeping Manager stated the faucet should not be unattached and that hot water should be available, and also stated she was unaware of the issue in the room. The Maintenance Director stated the faucet should be attached to the sink and that hot water should be accessible, and said staff were expected to document maintenance issues in the maintenance logs but did not. Review of the June 2026 Maintenance Repair Log did not show an entry for the faucet being unsecured and without accessible hot water in room [ROOM NUMBER]. The facility policy titled Maintenance service, dated December 2009, stated maintenance personnel are responsible for maintaining plumbing fixtures in good working order, and the DON confirmed the policy was not followed.
