Summary

A working call system was not available in a resident’s bathroom and bathing area, and the facility failed to ensure a call light was functioning for one of six residents observed for physical environment. During an unannounced complaint visit, Resident 1, who had diagnoses including dysarthria following cerebral infarction and hemiplegia and hemiparesis affecting the dominant side, stated he had been pressing his call light for about 30 minutes and no one came. He said he needed to be changed and then uncovered his blanket to show a wet diaper soiled with urine. At the time of the observation, Resident 1 was seen pressing the call light button, but no staff came to the room for another 15 minutes. The resident’s MDS showed a BIMS score of 99, indicating the assessment could not be completed, and section C0700 indicated his short- and long-term memory was okay. The Maintenance Director stated he checked residents’ call lights daily in two rooms per day and that the whole system was checked at the end of the month. He also stated he had already replaced Resident 1’s call light after the ADON asked him to check it, and he carried the defective call light that had no red top or push button. The ADON stated a working call light was important to meet Resident 1’s needs and ensure safety. The facility’s testing record showed the call light had been checked as okay on prior dates, and the facility policy stated residents should have a call system to allow them to call for staff assistance.
