Failure to Implement and Document Hypoglycemia Treatment
Summary
The facility failed to ensure physician-ordered hypoglycemia management was implemented and documented for two residents with diabetes mellitus. Resident 36 had orders for Humalog sliding scale insulin for blood glucose less than 70 mg/dL, with instructions to initiate hypoglycemia management and call the physician, as well as an order for glucose dextrose 4 tablets to be given once as needed for abnormal blood glucose when hypoglycemia was present and the resident could tolerate PO. On June 10, 2026, Resident 36 had a blood glucose level of 63 mg/dL, but there was no documentation showing that any intervention was provided or that the hypoglycemia protocol was followed. During interview and record review, RN 1 and RNS 1 stated that when a resident’s blood glucose is below 70 mg/dL, the licensed nurse should implement hypoglycemia interventions such as medication, juice, or snacks in accordance with physician orders and facility protocol. They stated there was no documentation showing what interventions were provided for Resident 36, and that the nurse should have documented the interventions performed and the resident’s response. RNS 1 also stated facility protocol required documentation of interventions and follow-up related to hypoglycemic events, and that no documentation could be located showing the event was managed. Resident 39 also had diabetes mellitus and orders for Humalog sliding scale insulin for blood glucose less than 70 mg/dL, with instructions to initiate hypoglycemia management and call the physician, along with an order for glucose dextrose 4 tablets as needed for hypoglycemia when the resident could tolerate PO. On June 9, 2026, Resident 39 had a blood glucose level of 61 mg/dL, but there was no documentation that interventions were provided or that the hypoglycemia protocol was initiated and followed. RN 2 and RNS 2 stated the licensed nurse was responsible for addressing blood glucose levels below 70 mg/dL by implementing ordered medications or providing juice per protocol, and that the resident’s low blood glucose was not documented as treated in accordance with the physician’s order.
