Late Dialysis Transportation and Poor Communication
Summary
The facility failed to implement the care plan for a resident who required hemodialysis and had diagnoses including acute respiratory failure with hypoxia and end stage renal disease with hemodialysis dependence. The resident’s MDS showed intact cognition with a BIMS score of 14 and significant assistance needs for daily activities. The resident stated there had been repeated problems with transportation for dialysis, including late ambulance pickup and late return to the facility, and said the dialysis center had him sign paperwork because his treatment was shortened. On 5/13/2026, the resident was still in his room waiting for transportation for HD and stated he was running late again and had not been told anything. The EMT stated dispatch provided pickup and destination information but not an exact pickup time. The case manager stated the resident’s HD start time was 9:30 a.m. with a pickup time of 8:30 a.m. or 9:00 a.m., but did not know the physician’s order dated 4/29/2026 specified an 8:15 a.m. pickup time. The case manager also stated the resident was being late and the HD time would be shortened. Record review showed the dialysis communication record did not include the time HD started and ended, and the LVN stated the facility usually notifies the dialysis center if transportation is late but was not aware the resident’s dialysis was cut short. An AMA form dated 5/11/2026 documented the dialysis treatment was shortened by 24 minutes from the prescribed 195 minutes because the resident arrived late. The DON stated the case manager did not follow the resident’s care plan or order for dialysis transportation, and that the facility should have communicated with the case manager and dialysis center when the resident was late and should have been alerted when the dialysis treatment was cut short.
