Undated and Expired Food Stored in Kitchen and Resident Refrigerator
Summary
Safe and sanitary food storage and preparation practices were not maintained in the kitchen when multiple food items were found stored without required dates or beyond their allowed storage periods. During observation, three bags of frozen chicken were in the reach-in freezer with no date or label, and the Assistant Dietary Supervisor stated the chicken had been removed from its original packaging and should have been marked with the date it was opened or delivered. In the walk-in refrigerator, an open container of chicken tortilla soup was found with a use-by date of 5/9/2026 and was identified as expired and left in storage. Five bags of thawed raw chicken with a use-by date of 5/6/2026, three logs of ground beef with a use-by date of 5/10/2026, and two large bags of marinated beef chunks with a use-by date of 5/8/2026 were also observed in the walk-in refrigerator. The ADS stated the chicken and ground beef had been stored longer than 5 days, the chicken was past the use-by date, and the marinated beef was staff food that should not have been stored in the facility refrigerator. Nutrition shakes stored in the kitchen were also not dated after thawing. Two boxes of chocolate-flavored nutrition shakes and one box of strawberry-flavored nutrition shakes were found in the walk-in refrigerator with no thaw date, even though the manufacturer’s instructions stated the shakes were to be used within 14 days of thawing. The ADS stated the shakes had been delivered frozen and then stored in the refrigerator, but staff did not know when they had been thawed because no thaw dates were present. The ADS acknowledged the shakes were dairy based and stated residents could get sick if they received expired shakes. A resident refrigerator located in the first-floor utility room also contained undated items. One thawed nutrition shake, one renal nutrition supplement, and one soda were observed with no date or label. The ADS stated the nutrition shake was from the facility and was only good for 14 days once thawed, while the ADON stated all food and beverages stored in the resident refrigerator had to have a date and label indicating discard within 2 days. The DSD stated nursing staff were expected to label and date leftovers placed in the resident refrigerator, and that undated food could make residents sick.
