Fortified Diet Orders Not Followed During Meal Service

The Rehabilitation Center Of Los AngelesLos Angeles, California Survey Completed on 05-14-2026

Summary

The facility failed to provide fortified diets for 18 of 112 meal trays that required them. During lunch tray line service, meal trays marked for fortified diets received the same food as trays without fortified diet orders. A dietary aide did not communicate the fortified diet orders written on the meal tray cards to the cooks, and the cook did not add any additional food items to the trays. The dietary aide stated fortified diets were meant to add extra calories for residents who were losing weight, but acknowledged that the fortified diets were not communicated to the cook and that no one received anything extra. The cook confirmed that fortified diets were supposed to include added items such as melted butter, but none was added during the lunch service because the dietary aide did not read out the fortified diet orders. The registered dietitian stated fortified diets were recommended for residents at risk of weight loss and required an additional 500 calories per day spread over three meals, with options such as melted butter over vegetables and starches, high protein and calorie mashed potatoes, or creamy soup. Facility guidelines also stated that fortified foods were to be provided within the specifications of the diet order and that fortified enhanced power foods were intended to increase calories and/or protein in meals.
