Missing care plans and failed implementation for pneumonia, diabetes education, and hemodialysis transportation
Summary
Resident 4 did not have a comprehensive care plan developed for pneumonia-related antibiotic use after the resident developed a fever and tachycardia and then received an order for Ertapenem Sodium 1 gram IV daily for pneumonia for 10 days. The record review showed the resident had diagnoses including gastrostomy tube dependence, chronic respiratory failure, and ventilator dependence, and the DON confirmed that when a resident started an antibiotic medication, nursing staff had to develop a care plan for the diagnosis of the antibiotic use. The review found that a care plan for antibiotic use and a care plan for pneumonia were missing when the antibiotic was ordered. Resident 169 had diagnoses including essential hypertension, type 2 DM, CKD stage 4, PVD, and a right 3rd toe ray amputation. The resident’s MDS showed cognitive intactness and that Spanish was the primary language. During observation, the resident stated he was supposed to have food low in sugar but was served sweet juice and ice cream and had too many finger sticks and insulin injections. The chart review and interviews found a care plan intervention for diabetes management, including meal planning and education, but there was no documentation that the resident was educated on diet or that noncompliance with the diet was documented. The SBAR also did not show documentation of diet noncompliance or education. Resident 190 had diagnoses including acute respiratory failure with hypoxia, ESRD, and dependence on hemodialysis. The resident stated transportation for HD was repeatedly late, including on one occasion when the ambulance arrived late and the dialysis session was shortened, and on another occasion when the resident was still waiting for transportation and no one had informed him of the delay. The CM who scheduled transportation stated the HD start time was 9:30 a.m. with a pick-up time of 8:30 a.m. or 9:00 a.m., but did not know the physician’s order required an 8:15 a.m. pick-up time. The record included an AMA form showing the dialysis treatment was shortened by 24 minutes because of the late arrival, and the DON stated the CM did not follow the resident’s care plan or order for dialysis transportation.
