Incomplete Response to Resident Council Call Light Grievances
Summary
The facility failed to ensure grievances and recommendations voiced through Resident Council meetings were acted upon and responded to for two residents who attended a meeting with the nurse surveyor. The deficiency involved repeated resident council concerns about delayed response to call lights that were not fully investigated or documented in the grievance process as required by the facility’s policy. Resident 75 was admitted with diagnoses including dysarthria, cerebral infarction, and DM. The resident’s MDS dated 2/6/2026 indicated intact cognition for daily decision-making and dependence on staff for ADLs including bed mobility, transfers, eating, walking in the room, dressing, toileting, and personal hygiene. Resident 147 was originally admitted in 2015 and re-admitted later with diagnoses including COPD, CKD, and generalized muscle weakness. The resident’s H&P dated 11/3/2025 indicated capacity to understand and make decisions, and the MDS indicated moderately impaired cognition with varying levels of assistance needed for ADLs. Resident Council Minutes documented repeated complaints about long call light wait times on multiple shifts, including concerns raised about the 11 PM-7 AM shift, the 3 PM-11 PM and 11 PM-7 AM shifts, and the 3 PM-11 PM shift. Corresponding grievance/complaint resolution reports showed entries such as grievance received, 1:1 in-service started, managers notified, or education needed, but several reports lacked a summary of pertinent findings or conclusions, and one report was blank in that section. During the resident council meeting on 5/11/2026, Resident 75 and Resident 147 stated the facility had been made aware of call light delays and that the issue remained ongoing and unresolved. Facility leadership reviewed the records and stated the grievance documentation was incomplete because it did not include the full investigation, the findings or conclusion, whether the grievance was confirmed, and the corrective action taken.
