Inaccurate Skin Assessments and MDS Documentation

Summary

Resident 1’s assessments were not completed accurately on admission. The admission MDS dated [DATE] listed multiple diagnoses, including generalized muscle weakness, type 2 diabetes mellitus, and CVA, but Section M did not reflect the stage 2 pressure ulcers on the right and left buttock that were documented in the hospital record and care plan. The DON and MDSC confirmed the hospital record showed a back wound and wound care involvement, and the care plan dated 5/26/24 identified hospital-acquired stage 2 pressure ulcers to both buttocks, yet the admission MDS did not capture those wounds. The admission nursing assessment also did not accurately reflect Resident 1’s skin integrity. The DON confirmed the Nursing Documentation Evaluation dated 5/25/24 indicated no skin issues, despite the care plan identifying scattered discoloration to the left and right upper extremities and stage 2 pressure ulcers to both buttocks. The DON stated the assessment was inaccurate and that nurses should have completed an accurate head-to-toe skin assessment, documented the wound condition, notified the physician, obtained treatment orders, and requested a wound consult. The DON also confirmed there were no treatment orders for the stage 2 pressure ulcers until 6/5/24. Resident 1’s weekly nursing assessments were also inaccurate. The DON confirmed weekly summaries dated 5/30/24 and 6/2/24, as well as the daily documentation dated 6/5/24, indicated no skin issues. The DON stated these assessments were not accurate. The record further showed wound progress notes dated 6/6/24 and 6/20/24 indicating the right and left buttock stage 2 pressure ulcers merged into a stage 3 pressure ulcer at the coccyx on 6/20/24. The facility policy required comprehensive admission assessment and identification of skin integrity status through review of assessment information.
