Medication Administration Did Not Follow Ordered Parameters and Route

Summary

The facility failed to ensure medications were administered according to physician orders for one resident with diagnoses including surgical aftercare following genitourinary surgery, abnormal uterine and vaginal bleeding, and female genital prolapse. The resident’s care plan dated 6/4/26 directed staff to administer medications as ordered, monitor vital signs as indicated, and notify the physician of abnormal readings. During interview and record review, the Nursing Supervisor confirmed that the resident’s June 2026 MAR showed Amlodipine, Losartan, and Metoprolol were administered on multiple days even though the documented blood pressure and pulse readings were below the ordered hold parameters. The MAR showed Amlodipine was given on several days when the resident’s blood pressure was below the ordered threshold of 140/80. Losartan was also administered on days when the resident’s blood pressure did not meet the hold parameters, and Metoprolol was given on multiple days when the resident’s blood pressure was below the ordered threshold and, on one day, when the pulse was 56. The NS stated the nurse should have held the blood pressure medications when the resident’s systolic BP was below 140, diastolic BP was below 80, and pulse was below 60, and the DON stated staff were expected to check parameters before administering blood pressure medications and follow the set parameters. The facility also failed to administer Estradiol vaginal cream correctly for three days. The MAR showed the cream was administered vaginally from 6/5/26 through 6/7/26, while the DON stated the resident’s hospital discharge records indicated the cream should have been dabbed on the vaginal area and that the word "dab" was not transcribed into the EHR order. Progress notes showed the resident had moderate vaginal bleeding on 6/9/26, the urogynecology office was contacted, the resident’s family member took the resident to the ED due to continued bleeding, and the resident was later readmitted from the hospital for abnormal vaginal bleeding.
