Summary

The facility failed to follow its change of condition policy for Resident 1 after the resident eloped from the facility between 2:00 AM and 3:00 AM on June 11, 2026. Resident 1’s face sheet showed diagnoses including encephalopathy and epilepsy. The change of condition report stated the resident was picked up and taken to the hospital, and the hospital called the facility around 5:00 AM and spoke with an RN. The RN supervisor informed the charge nurse that the hospital had called and that the resident was there, and the charge nurse notified the daughter. The morning nurse was also notified. During interview, LVN 1 stated that around 4:00 AM a CNA reported Resident 1 could not be located, and staff searched inside and outside the facility after the resident was discovered missing. LVN 1 stated she completed the change of condition report and estimated the elopement time based on the CNA’s last contact with the resident. She further stated that after 5:00 AM, the RN supervisor told her the hospital had called and reported the resident had been found and transported there. LVN 1 stated she notified the resident’s family at approximately 6:30 AM but did not communicate with the physician. The RN supervisor stated he instructed LVN 1 to complete the report and notify the family and physician, but he did not contact them himself because he had already directed LVN 1 to do so. The DON confirmed that LVN 1 did not comply with the policy by not informing the family as soon as the resident was found missing and by forgetting to notify the physician.
