Summary

The facility failed to ensure that a resident with a suprapubic catheter had the catheter changed monthly as ordered by the physician. Resident 9 was admitted and later readmitted to the facility with diagnoses including obstructive reflux uropathy, urinary retention, and dementia. The resident’s H&P stated the resident could make needs known but could not make medical decisions, and the MDS indicated the resident had an indwelling catheter. A physician order dated 3/8/2026 directed that the suprapubic catheter be changed monthly and as needed, and the care plan included an intervention to change the catheter as ordered. During interview, the Treatment Nurse stated the last catheter change occurred in March 2026 when the resident was sent to the hospital, and that the catheter had not been changed for two months. The nurse stated this was an oversight and acknowledged the importance of changing the suprapubic catheter on a regular monthly schedule as ordered to prevent UTI, damage, and accumulation of urine sediments. The Registered Nurse also stated that regular catheter changes are important to prevent UTI and maintain patency of the tube. The facility policy stated suprapubic catheter care shall be provided according to physician orders, professional standards of practice, the resident’s condition, care plan, and infection control policies.
