Summary

The facility failed to ensure that a cardiology referral was completed for one resident with a history of chest pain, a cardiac pacemaker, and dementia. The resident’s admission record listed these diagnoses, and the Minimum Data Set dated 4/23/2026 indicated the resident was independent in cognitive skills for daily decision making and required supervision with toileting hygiene, upper and lower body dressing, and personal hygiene. Progress notes dated 5/18/2026 documented a referral to another cardiologist for continuation of care because the resident’s previous cardiologist was not covered by insurance.
