Summary

The facility failed to ensure the Minimum Data Set (MDS) was coded accurately for one resident, identified in the report as Resident 47. The resident’s record showed admission and readmission to the facility, with diagnoses including personal history of other mental and behavioral disorders, HTN, and BPH. A PASARR Level 1 Screening dated 10/16/2025 identified schizophrenia as a serious mental illness and indicated the case was referred for PASARR Level II review. A review of the resident’s History and Physical dated 5/23/2026 stated the resident did not have the capacity to understand and make decisions. During a concurrent interview and record review on 6/3/2026, the MDS Nurse stated the resident’s MDS was completed inaccurately and that Section A1510 should have been checked for serious mental illness, with no check in the category for other related conditions. The nurse stated the resident’s schizophrenia is considered a serious mental illness and that the inaccurate MDS assessment would result in inappropriate psychiatric intervention and delay of care and services. The facility policy titled Certifying Accuracy of the Resident Assessment stated that any person completing a portion of the MDS must sign and certify the accuracy of that portion of the assessment.
