Hand Hygiene and Glove Change Not Performed During Nephrostomy Dressing Change

Summary

The facility failed to practice appropriate infection prevention and control measures during a nephrostomy tube dressing change for one resident who was on Enhanced Barrier Precautions. The resident was admitted with diagnoses including artificial openings of the urinary tract and had a right flank nephrostomy tube. The treatment administration record directed staff to cleanse the nephrostomy tube site with normal saline, pat dry, and apply a split gauze dressing daily and as needed if soiled or dislodged. The care plan identified the resident as high risk for complications including UTI due to the nephrostomy tube and included interventions to implement EBP and observe good hand-washing technique. During observation, a licensed nurse entered the resident’s room to change the nephrostomy dressing, donned gloves without performing hand hygiene, and touched the light fixture and curtains in the room. The nurse removed the soiled dressing, cleaned the nephrostomy tube site, and applied a new dressing without performing hand hygiene or changing gloves during the procedure. The nurse confirmed that only one set of gloves was used for the entire treatment. The infection preventionist stated staff should perform hand hygiene and don gown and gloves for high-contact activities under EBP, and that gloves should be changed after removing the soiled dressing and again before placing the new dressing. The DON stated the nurse should have changed gloves during the dressing change. The facility policy titled Wound Care included washing and drying hands thoroughly after removing the dressing and before putting on gloves again.
