Summary

The facility failed to ensure dental services were provided to Resident 24, who had diagnoses including major depressive disorder, type 2 diabetes mellitus, anemia, and dysphagia. The resident’s records showed an onsite mobile dental form dated 10/25/2025 indicating the resident wanted the dental treatment recommendations completed, and the order summary dated 3/9/2026 listed dental consultation as needed with treatment to be completed as indicated by the resident’s condition. The MDS dated 3/13/2026 indicated the resident had no natural teeth or tooth fragments, needed partial/moderate assistance with oral hygiene, and required supervision or touching assistance when eating. The care plan dated 4/10/2026 identified dental health problems, missing teeth, and that dental care consultations should be coordinated as needed. During observation and interview on 5/28/2026, Resident 24 had some missing and broken teeth and was not wearing dentures. The resident stated some natural teeth were gone, some were broken, and the resident had not been seen by a dentist for a long time, despite telling staff about needing a dental consultation. CNA 3 stated the resident had missing and broken teeth and took additional time to chew food. The SSD and DPC stated the resident was not provided dental services, and the DON stated the resident should have been seen by the dentist. The DON also stated Social Services should have arranged the dental services and that dental consultations were provided as needed, monthly, quarterly, or every six months. The facility policy titled Dental, vision, and hearing evaluations stated dental evaluations are done to assure residents can function at their highest practical level and include assessing the need, obtaining a physician’s order, and arranging the evaluation as soon as possible.
