Kitchen Food Storage, Cutting Board, and Emergency Water Labeling Deficiencies

Temple Park Convalescent HospitalLos Angeles, California Survey Completed on 05-29-2026

Summary

The facility failed to follow sanitary food and water safety practices in the kitchen by leaving an opened container of blueberry yogurt in Refrigerator 2 without an open date or use-by date. During observation, two of three cutting boards in the dry food preparation area had deep knife marks, and the Dietary Supervisor stated the yellow cutting board had knife marks that were easily felt. Kitchen staff stated that opened food items must be labeled with the open date and use-by date, and that cutting boards should be replaced when they have visible cuts or feel rough to the touch because bacteria and small plastic particles can transfer to food during meal preparation. The facility also failed to label 13 five-gallon emergency water containers in kitchen storage as potable water for human consumption. The containers were observed without delivery dates and without any indication that they were safe for drinking, food preparation, or cooking during an emergency. The Maintenance Supervisor stated the containers were designated as potable water for resident and staff consumption and should have been labeled and documented with the delivery date, expiration date, and supplying company. The Dietary Supervisor stated it was best practice for the emergency water containers to be labeled with the date received to ensure proper tracking.

The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Ice Machine and Water Filter Not Properly Cleaned: An observation found black and brown substance on the ice machine's water curtain guard and a dark brown water filter behind the machine. The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
